On December 28, 1978, the cockpit flight crew of United Airlines Flight 173 became distracted by a potential landing gear issue and failed to properly monitor the aircraft’s fuel state. While holding for approximately one hour, with the landing gear extended, the co-pilot and flight engineer did not adequately communicate their concern regarding the diminishing fuel supply to the captain. The captain, a highly respected 27-year United veteran, did not properly respond to the low fuel statements made by his cockpit crew. The aircraft eventually lost all engines due to fuel starvation and tragically crashed killing 10 and seriously injuring another 23 onboard.
This accident is considered by many to be one of the most important in aviation history, as it led to the dawning of a new type of pilot training known today as Crew Resource Management (CRM). Now a requirement for airlines, military aviation and NASA, CRM stresses crew communication, teamwork, and other critical human factor skills above the historical rank hierarchy that led to the 1978 disaster. Since its inception, CRM has also been introduced to the medical community, oil and gas industry, nuclear power industry and specialized firefighting teams.
